Our Wellness Nurse (RN) provides high‑quality care through health assessments, medication management, and coordination with residents, families, and the care team. The role supports the Health and Wellness Director and ensures residents receive consistent, safe, and compassionate care.
Key Responsibilities- Perform resident assessments and reassessments according to policies and monitor any changes in condition.
- Communicate with the Health and Wellness Director about resident needs to ensure consistency in quality of services.
- Participate in hands‑on care, medication administration, and specific nursing tasks when necessary.
- Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines.
- Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
- Assist in the transcription of physicians' orders and review new physician orders.
- Maintain accurate and up‑to‑date resident records, including care plans, incident reports, and medical documentation.
- Coordinate care planning with home‑health agencies on‑site, working within community and company policies.
- Address residents’ special needs, requests, and complaints.
- Coordinate admissions with the sales team, including supervising move‑ins.
- Understand community care regulations and state‑specific regulations.
- Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
Education and
Certifications:
- Valid, incumbent nursing license (RN) required.
- High School Diploma or equivalent.
- Associate degree or higher in nursing preferred, or a combination of education and experience. Additional coursework in management is an asset.
- Current First Aid and CPR license or ability to obtain.
- At least four (4) years of geriatric nursing experience, including medication administration, delivery systems, and pharmacies.
- At least one (1) year of supervisory experience, including staff scheduling and management (preferred).
- Experience with memory care is a plus.
- Excellent attention to detail, integrity, communication skills, teamwork, and leadership.
- Outstanding organizational skills and positive attitude, empathy, patience, and respect for residents.
- Willingness to participate in on‑call rotation.
- Must meet all mandatory health requirements according to state regulations.
Salary: $50 - $51.92 per hour.
