• Perform member outreach via phone or home visits to engage members regarding their care/service plans
• Coordinate care activities based on care plans and collaborate with healthcare providers and community partners
• Address member and provider inquiries related to care plans and services
• Document and maintain member records in compliance with state and regulatory requirements
• Provide education on available benefits and resources to members

• Requires a High School diploma or GED
• Minimum of 1-2 years of experience in a related field
• Phone customer service experience
• General computer knowledge, including proficiency in MS Office (Excel, Word, Outlook)
• Experience in managed care or a physician’s office setting

• Bachelor’s degree preferred, but not required
• Experience directly working with healthcare providers or patients
• Scheduling experience in a healthcare setting
• Familiarity with health needs assessments and call center metrics
