Behavioral Health Specialist, Community Case Manager

• Provide therapeutic intervention to assigned caseload of patients based on individualized treatment plans
• Assist clients in achieving personal independence and developing daily living skills
• Facilitate access to community support systems and linkages to formal services
• Participate in interdisciplinary team meetings to promote patient success and implement interventions
• Document therapeutic interventions and patient responses accurately in the electronic medical record

• High school diploma or equivalent
• Minimum of 2 years of experience in a related job discipline
• Active Ohio driver's license with fewer than five points on your record
• Ability to provide transportation for clients using a personal vehicle
• Strong critical thinking skills for implementing therapeutic interventions

• Bachelor’s Degree in a related field
