About Waterfall Community Health Center

Waterfall Community Health Center (WCHC), located on the Oregon Coast, provides integrated patient-centered primary care to a diverse population with emphasis on serving individuals who face barriers to care. The mission is to promote access to quality integrated health services that meet community needs on the Southern Oregon Coast.

Responsibilities
  • Adhere to WCHC’s mission and core values (Integrity, Respect, Accountability, Compassion, Partnership, Collaborative Communication).
  • Follow the American Counseling Association (ACA) code of ethics and apply a strengths-based framework.
  • Provide trauma-informed outpatient counseling with evidence-based assessments and interventions.
  • Facilitate individual and/or group counseling using evidence-based approaches for a range of mental health needs.
  • Coordinate referrals, address social determinants of health, and collaborate with community partners within an integrated care team.
  • Assess behavioral and mental health status using patient information, interviews, observations, tests, and collaboration with the care team.
  • Maintain accurate records and documentation per clinic policies and regulatory guidelines.
  • Build rapport with a diverse patient population, including those with developmental conditions, psychiatric diagnoses, and substance use concerns.
  • Work independently while engaging in proactive patient panel management and timely service graduation.
  • Support a trauma-informed, growth-oriented environment for patients, staff, and the community.
  • Advocate for patients to coordinate required services and manage crisis situations.
  • Develop and implement treatment plans with measurable objectives based on DSM-5-TR standards.
  • Collaborate with Psychiatric and Primary Care Providers to address biopsychosocial issues.
  • Communicate professionally with patients via phone, MyChart, and other channels.
  • Adapt communication strategies to stabilize or de-escalate high-stress situations with appropriate referrals.
  • Maintain confidentiality and HIPAA compliance in all clinical activities.
  • Support program goals to reduce cultural and socio-economic barriers for marginalized communities.
  • Potential opportunity to become a clinical supervisor with adjusted reimbursement and supervision.

Required Skills / Abilities
  • Excellent verbal and written communication.

  • Experience with Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
  • Experience with Electronic Health Records.
  • Ability to pass a criminal background check.
Education and/or Experience & Qualifications
  • Master’s degree in counseling from an accredited university.
  • Unrestricted Oregon LPC license.
  • Certifications in EMDR, EFT, ACT, Trauma-Focused CBT, TCT, or DBT encouraged.
  • Non-violent Crisis Intervention training completed within 6 months of hire and recertified before expiration.
  • Preferred: 1+ year healthcare or behavioral/mental health experience.
  • Preferred:
    Experience serving individuals with socioeconomic needs.
  • Preferred:
    Bilingual (Spanish).
  • Preferred:
    Current First Aid or BLS certification or completion within 6 months of hire.
Immunizations
  • TB test required upon hire.
Physical Requirements
  • Prolonged periods of sitting/standing at a desk; computer work.
  • Able to lift up to 15 pounds occasionally.
  • Able to travel as needed.
