• Call patients to check in after discharge and ensure they have received the necessary assistance
• Coordinate services and care for members with affiliated vendors, including transportation and home health care
• Assist with scheduling post-discharge appointments with primary care and specialist providers
• Collaborate with patients, providers, and caregivers to ensure positive health outcomes
• Participate in and assist with pilot programs and interdisciplinary huddles to address patient needs

• Minimum of 1 year of experience working in the Transitions of Care (TOC) program
• Medical Assistant Certification
• 2-3 years of experience in healthcare, managed care, home health, and/or community health setting

• Fully bilingual in English and Spanish
