• Develop and implement personalized LTSS care plans for members and their families
• Engage with members and families to inform them about LTSS Community Partner supports
• Collaborate with care teams to deliver a person-centered, comprehensive plan of care
• Link members to social services and community resources to address medical and non-medical needs
• Provide health and wellness coaching to help members achieve personal health goals

• LICSW or LCSW; or Bachelor's degree in social work, human services, nursing, psychology, sociology, or related field; or Associate's degree with at least one year of professional experience; or at least three years of relevant professional experience
• Pediatric and/or Behavioral Health experience strongly preferred
• Ability to conduct face-to-face appointments with members and providers
• Advanced skills in consumer assessment and the ability to assess physical conditions of consumers' homes
• Proficient in reading, writing, and communicating in English

• Experience working with Medicaid recipients and community services
• Experience with Epic, eHana, or other EHR systems
• Bilingual skills (e.g., Spanish, Haitian Creole, Cape Verdean Creole)
