The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Habilitation Supervisor II. The Habilitation Supervisor II is responsible for supervising and providing leadership, mentoring, coaching, monitoring and on-the-job training to supervisors and staff who provide direct care in a 24 hour residential State Operated Group Homes.
The Habilitation Supervisor II serves as a role model for supervisors and direct care staff.
- Supervises supervisors and direct care staff; completes performance appraisals and ensures staff time sheets submitted timely; facilitates shift meetings; completes work schedules; participates in hiring interviews; problem solves issues. Communicates pertinent information to staff related to their job and the individuals for whom they are responsible.
- Trains and monitors staff from all shifts to assure implementation and compliance with federal ICF/IID regulations and state Arizona Administrative Code (AAC) Title 6, Chapter 6, Article 8; counsels staff regarding work roles and responsibilities by implementing the active treatment plan inclusive of the skill building, therapeutic and behavioral building plans.
- Participate in the development of the PCSP, PCSP reconvenes, Behavior Management Reviews, Psychiatric clinics, Home Risk Meetings, etc. Ensure staff are knowledgeable of each member and able to advocate for the member effectively. Review strategies, data records, mealtime books, monthly progress reports, etc. for accuracy and completeness.
- Coach/mentor/teach staff to comply with the PCSP plan regarding programs, activities, therapy needs, behavior challenges, active treatment and mealtimes. Ensure provision of socialization and positive learning experiences in the community. Administer medications properly upon successful completion of appropriate 16-hour medication technician or medication administration course & monitor to assure all staff follow procedures.
- Demonstrate leadership by displaying a positive attitude, promoting teamwork, ensuring compliance with policies and procedures; problem solving; conducts monthly staff meetings; giving recognition and striving to improve services.
Knowledge in:
- Agency rules, regulations, policies and procedures relating to residential care for individuals with developmental disabilities
- Federal Title XIX ICF/IDD regulations
- Person Centered Service Plan (PCSP) process
- Members' rights and advocacy
- Specialized needs for individuals with developmental disabilities
Skills in:
- Oral & written communication, listening and problem solving
- Multi-tasking and prioritizing; and Coordinating and monitoring the implementation of prescribed therapeutic routines
- Time management, motivation, teamwork and team building
- Interviewing and supervisory techniques
- Collecting data for member program planning and Mathematical calculations
Ability to:
- Evaluate staff performance and resolve problems
- Manage a diverse group of staff to develop goals and teamwork to accomplish those goals
- Manage emergency situations in an effective and professional manner
- Monitor the provision of care to individuals, identify problems and implement solutions
- Promote good communication
- Must be at least 18 years of age and be able to read and write in English.
