GENERAL SUMMARY

The Intake Specialist is responsible for all aspects of the initial assessment of members including interviewing, gathering data for a comprehensive psychosocial assessment, making a clinical diagnosis, and completing the interim service plan. The Intake Specialist also assists with completing SMI Determinations, substance use assessments, and crisis intervention. Interacts professionally and clearly and in a timely manner with internal and external customers and other community professionals.

RESPONSIBILITIES
  • Conduct a comprehensive psychosocial assessment and make treatment recommendations, summarize primary presenting issues, identify child/family strengths, and formulate a diagnosis based on this initial interview.
  • Build rapport and credibility simultaneously with members. Relate to a variety of individuals with warmth, empathy, and acceptance.
  • Develop interim service plans that identify the next steps taken after intake by the provider and person/family, and what issues are to be addressed for ongoing assessment.
  • Provide effective crisis assessment including following the program triage protocol, arranging for emergency psychiatric evaluation/hospitalization as indicated by protocol, and providing follow-up, normalizing the situation, discussing options, and handling the situation calmly.
  • Conduct educational activities that promote member understanding of treatment services, including therapy, behavior management, psychiatric evaluations, medication, diagnosis, and parenting skills.
  • Perform duties with knowledge and skill in group processes, establish group commitment to common goals, and create an environment of mutual respect and understanding.
  • Perform duties utilizing knowledge and skill in assessment and diagnosis using DSM‑V criteria.
  • Understand human development, family life cycles and demonstrate application of this knowledge in assessment and diagnosis.
  • Maintain clinical records in a timely manner according to standards.
  • Attend all intake department staff meetings, participate in the process of improving admissions procedures, and assess the quality and effectiveness of intake services.
  • Participate in in‑service training and other educational opportunities as required and as time and resources permit.
  • Complete SMI Determinations for new enrolled members and existing members.
  • Assist with crisis intervention techniques.
  • Perform other duties as assigned by the Program Director.
  • Follow policies, procedures, and controls established by the organization and HIPAA officers regarding PHI.
  • Perform other related duties as assigned or necessary relative to the position.
QUALIFICATIONS
  • Education and Experience – Completion of a Master’s Degree in Social Work, Counseling, Therapy, Psychology, or Education from an accredited school highly preferred; or completion of a related Bachelor’s degree program and two years full‑time work experience in the behavioral health field, or a non‑related bachelor’s degree with four years of full‑time experience in the behavioral health field.
  • License – Arizona Licensure (LISAC, LMSW, etc.) highly preferred.
REGULATORY
  • Minimum 18 years of age.
  • Arizona Driver’s License and 39‑month Motor Vehicle Report with proof of vehicle registration and liability insurance that meets insurance requirements, if required for position.
  • Must possess and maintain Level 1 fingerprint clearance throughout employment.
  • CPR and First Aid certification, if required.
