• Provide direct care and support to participants in daily living activities
• Assist with personal hygiene, health monitoring, and basic self-care routines
• Encourage participation in community activities and meaningful goals aligned with participant plans
• Collaborate with supervisors and care teams to ensure participant safety and well-being
• Document progress and communicate effectively with participants, families, and team members

What You’ll Bring To Sevita

• Education:
  
  High School Diploma or equivalent
• Experience:
  
  Six months of experience in human services, direct care, or care coordination preferred
• Skills:
  
  Communication, adaptability, multi-tasking, teamwork, time-management
• Behaviors:
  Patient, compassionate, reliable, responsible
• Vehicle:
  Valid Driver's license and access to a registered vehicle with proof of insurance
