As a
** Hospice Registered Nurse Case Manager,
** you’ll contribute to our success in the following ways:
* Completes routine and emergency “face to face” recertification visits with patients entering their 3rd benefit period or beyond, including admissions and transfers and extended length of stay.
* Records observations, treatments, and other pertinent information.
* Communicate with Medical Director, Attending Physician, branch leadership and IDG regarding findings from the visits and any measures to alleviate symptoms.
* Provides physical and emotional care to support the patient and family in period of crisis.
* Ability to triage consults by patient acuity.
* Performs bedside palliative care with patient/family education
* Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
* Prevents unwanted hospitalizations by meeting patient and family needs in the home.
* Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
* Compliance with applicable laws and regulations with respect to Collaborative Agreements.
* Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation.
* Provides training and continuing education for staff.
* Assists in development of clinical practice guidelines/standards in support of quality palliative care.
* Adheres to the practice of confidentiality regarding patients, families, staff and the organization.
* Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG.
* Performs other duties/projects as assigned.
* Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board.
* 1+ years of clinical nursing experience
* 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred
* Prior experience in Family Medicine, Geriatrics and psychiatry preferred.
* Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.
