Mental Health, Clinical Social Worker
Tri-County Behavioral Healthcare is seeking an experienced clinician to provide comprehensive intake assessments and determine eligibility for youth and families seeking care. This position focuses on continuity of care and coordination for youth involved in the juvenile justice system.
This position works directly with juvenile detention facilities, probation departments, courts, and community partners to ensure youth with mental health needs are identified quickly and connected to appropriate treatment and services.
The ideal candidate is a clinically strong, experienced clinician who is comfortable working with complex youth presenting with trauma, behavioral challenges, and justice system involvement.
What you’ll do- Perform continuity of care screenings and intake assessments for youth involved in the Criminal Justice system or in Juvenile detention.
- Complete diagnostic evaluations and complete clinical documentation to determine eligibility of services.
- Provide continuity of care planning and coordinate with internal programs and community partners.
- Provide case management to families and youth admitted into services.
- Assist youth and families who are not admitted by linking them with appropriate community resources.
- Collaborate regularly with juvenile probation, detention staff, law enforcement, courts, county officials, and community stakeholders.
- Assess and manage care for high risk youth with complex behavioral health needs.
- Maintain timely documentation and compliance with clinical, state, and Center standards.
- Demonstrate effective teamwork and contribute to a positive peer culture in alignment with the mission and values of the agency at all times.
- Master's Degree in the Social Services Field
- Social Work, Psychology, Counseling, Marriage and Family Therapy, or related field.
- State Licensure as an LPC, LCSW or LMFT
- Experience diagnosing mental impairments and recommending treatment
- Knowledge of mental health screening and assessment tools
- Ability to pass TDCJ NCIC background check
- Valid driver’s license and insurable through Tri-County.
- Must be attentive, engaged, and able to interact safely and effectively with individuals with mental health disorders and/or intellectual and developmental disabilities (e.g., depression, bipolar disorder, schizophrenia, autism spectrum disorder, developmental delays)
