Overview
TX Supported Home Living (SHL) role SHL Team member provides services for Home and Community Based Service (HCS) and Texas Home Living (TxHmL) program participants in their home or the community in compliance with the Person Directed Plan (PDP) and the Evidentiary Certification Principles.
Responsibilities- Provide one-to-one contact with the individual to assist with daily living and social skills
- Provide access to communication and information from the environment in a manner that is clear and understandable to the participant
- Form a relationship with the individual that recognizes, understands, and honors their attempts
- Develop and prepare activities for the individual
- Complete a work log of activities with the individual describing successful and unsuccessful techniques
- Provide technical assistance to others working with the individual
- Develop a working alliance with family, neighbors, and professionals with whom the individual has contact
- Participate in and assist in the development of the PDP and its implementation
- Document areas identified as needs in the PDP
- Report accidents and existing dangerous conditions and observe safety practices per the written plan for intervention with the family and IDT Team members
- Transport the individual to gain access to community services and resources required by the plan of care
- Provide instruction and support for the individual in: meal planning and preparation; self-care and personal hygiene; medications normally self-administered; household tasks; money management; safety and security; interpersonal communications; meaningful daily schedules and activity routines; mobility, ambulation, and exercise
- Provide behavioral support
- Ability to perform heavy lifting/carrying up to 75 pounds without assistance and 100+ pounds with assistance
- Perform health-related tasks that do not require standing physician orders
- Encourage teamwork, cooperation, trust, and a positive team identity; work cooperatively with others to achieve goals
- Request and use leave appropriately; dress appropriately; observe health, safety, and sanitation policies
- Follow all applicable rules of conduct and policies
- Able to recognize emergencies and implement emergency procedures
- Demonstrates respect for the patient, their property, and privacy
- Ability to maintain confidentiality related to agency activities, personnel actions, and individuals
- Knowledge and use of universal precautions and basic infection control procedures
- On-the-job experience that shows increased knowledge, skill, and abilities in providing services to more difficult individuals
- Ability to intervene with combative/aggressive individuals and participate in Handle with Care physical intervention
- Ability to perform Cardiopulmonary Resuscitation (CPR)
- High School Diploma or GED
