We  currently seeking a Licensed Practical Nurse (LPN) to join our growing team for on-call and per diem home visits.

• Provide skilled nursing care to clients in their homes under the direction of a Registered Nurse.
• Conduct timely and thorough assessments and deliver treatments based on physician orders and care plans.
• Educate clients and families on health conditions and self-care.
• Document all care activities in accordance with agency protocols and EMR requirements.
• Respond to client needs during assigned on-call shifts and per diem requests with professionalism and empathy.
• Collaborate with interdisciplinary teams to ensure continuity and quality of care.

• Licensed Practical Nurse (LPN) in the state of Connecticut
• 1+ year of nursing experience (home health preferred)
• Bilingual (Spanish/English) highly preferred to support communication with our diverse patient population
• Strong communication, critical thinking, and documentation skills
• Comfortable working independently in home settings
• Reliable transportation, valid driver’s license, and auto insurance
• Tech-savvy and familiar with EMR systems (training provided)
