Position Details:
Join Hartford Health Care’s thriving adult primary care practice in Bristol, CT—an easily accessible, central location just off I-84 and close to some of the area’s most desirable communities. This established, multi-provider office features an onsite pharmacist who helps manage complex diabetic and hypertensive patients. You’ll grow as an APP while working alongside an experienced, collaborative team and caring for a diverse patient population.

Setting : 100% Outpatient Primary Care – Adult Medicine (18+)
Schedule :
Full-time or part-time, weekdays only. 4-day workweek is an option!

Nurse Practitioners and Physician Assistants Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.
QualificationsRequirements :
- Licensure:
Unrestricted State of Connecticut Medical License (or eligible) - Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
- Experience:
2+ years’ experience in a primary care or relevant outpatient setting
