We are seeking a Registered Dietitian to provide patient nutritional care for a long term care / rehab facility in Conneaut, OH.
- Schedule:Part-time, approximately 8 hours per week
- Visits:Weekdays only
- Experience:Geriatric experience preferred, not required
Responsibilities include:
- Completing nutrition assessments and clinical documentation
- MDS assessments and care plan development
- Monitoring resident weights and participating in Nutrition at Risk meetings
- Conducting weekly meal rounds
- Completing monthly sanitation audits
Qualifications:
- Strong organizational and time-management skills
- Registration by the CDR (Commission on Dietetic Registration)
- State license or certification as applicable per state
- Criminal background check
- TB skin test
- Drug screen
Hourly Pay Rate is negotiable, with experience considered.
