Company DescriptionMGA Homecare has been providing Private Duty Nursing, Skilled Nursing Visits, Occupational Therapy, Physical Therapy, and Speech Therapy services within the home and community-based setting for over a decade. In 2022 MGA opened its doors to MGA Behavior Therapy offering ABA and early behavior intervention for pediatric patients with autism spectrum disorder (ASD) and other developmental disabilities within the Colorado Springs and surrounding areas.

MGA Homecare serves Arizona, Colorado, North Carolina, Tennessee, Texas, and Washington and is a resource for patients and their families.
We are seeking a compassionate and skilled RN or LPN to join our home health care team in Concord, United States. As a Home Health Nurse, you will play a crucial role in providing high-quality care to patients in the comfort of their own homes.
Responsibilities- Conduct initial and ongoing patient assessments to develop and update care plans
- Provide direct patient care, including wound care, medication administration, and vital sign monitoring
- Educate patients and their families on disease management, medication, and proper care techniques
- Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care
- Accurately document patient conditions, treatments, and progress in electronic health records
- Manage and organize daily visit schedules to maximize efficiency
- Adhere to all healthcare regulations and maintain patient confidentiality
- Participate in quality improvement initiatives and ongoing professional development
- Current RN license (Bachelor s or Associate s degree in Nursing) or LPN license (completion of an accredited practical nursing program) in the state of practice
- Valid CPR certification
- Strong clinical skills and knowledge of medical equipment and procedures
- Excellent communication and interpersonal skills
- Proven ability to work independently and manage time effectively
- Proficiency in electronic health record systems and documentation
- Experience in home health care preferred
- Understanding of home health care regulations and best practices
- Knowledge of common chronic conditions and their management
- Ability to adapt to various patient populations and home environments
- Valid driver s license and reliable transportation
- Additional certifications (e.g., IV therapy) may be required
- 1:1 care
- Compensation ranging from $60,652 to $105,534 annually
- Part time rates may vary
