Position Summary:
Representing the DALS Division around Clinical Operations: plan, coordinate, manage and evaluate the activities of a multidisciplinary team to ensure the delivery of high-quality home care services to the patients of his or her team. This will include oversight of the clinical team including nurse care managers and caregivers.
Essential Functions:
- Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.
- Provides guidance and counseling to continually improve all aspects of home health care services, provided through organization personnel.
- Promotes a culture of Service Excellence.
- Manages clinical teams and planning.
- Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
- Assists the Executive Director/Administrator in the preparation and administration of the organization's budget.
- Participates in performance reviews of Clinical Staff.
- Hires, evaluates, and terminates organization personnel.
- Assists in overseeing the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
- Assures proper maintenance of clinical records in compliance with local, state and federal laws.
- Develops implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, either directly or by delegating to another staff member.
- Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of home health services provided through the Organization.
- Assists in the development of organization goals. Develops, recommends, and administers organization policies and procedures.
- Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to Home Care standards.
- Monitors care and services, collecting and utilizing data to measure performance in comparison to National, State, industry benchmarks (if those benchmarks are available).
- Utilizes feedback from staff, residents, clients, and relevant others to improve quality.
- Investigates events, develops/implements action plans for resolution/improvement, and ensure reporting to regulatory agencies of all incident reports.
- Monitors adverse events.
- Utilizes structured approaches and tools to problem solving e.g. root cause analysis.
- Designs approaches which will improve measurable quality outcomes.
- Participates in Caring Community initiatives and is the Risk Manager for DALS.
- Stays informed about changes in the field of home health care; shares information with appropriate organization personnel.
- Promotes home health referrals in the health care community.
- Maintains a caseload of clients as needed to meet the needs of the organization.
- In the absence of the Executive Director/Administrator, the Clinical Director will become the acting Executive Director/Administrator and will be vested with authority to act in behalf of the Executive Director/Administrator.
Qualifications:
- Experienced in a clinical field (RN, LPN, PT, OT, SLP, SW).
- Maintains a current Massachusetts clinical license in good standing.
