Base pay range $58,240.00/yr - $66,560.00/yr.

Long Term Care Experience Required.

License in Social Worker – preferred but not required.

• Oversee the operation of the nursing center Social Services Department according to the NASW Code of Ethics while maintaining compliance with federal, state and local guidelines, policies and procedures, and standards of care for specialty practice.
• Foster a climate, policies and routines that empower patients and residents to maximize their individuality, independence and dignity.
• Ensure psychosocial assessments are completed for each patient and resident to identify social, emotional and psychological needs and strengths.
• Ensure patient and resident discharge goals are identified at admission and documented.
• Educate staff regarding the role of Social Services and the psychosocial needs of patients/residents and their families or significant others, including aging and disability issues.
• Measure outcomes and departmental productivity.
• Directly supervise two or more Social Services staff.
• Assess patients and residents for discharge and establish timely discharge goals.

• Bachelor’s degree in Social Work or Human Services required.
• Certifications/licenses required to practice in long‑term care, as mandated by state law.
