Facility: Concord Post Acute
Location: Concord, CA
Compensation: $17.00 per hour
Role Overview
Are you creative, energetic, and passionate about enriching the lives of seniors? Concord Post Acute is looking for a Full-Time Activities Assistant to help us run a vibrant, interactive, and meaningful activities program. Working under the direction of the Activity Director, you will play a key role in ensuring our residents physical, mental, and psychosocial needs are met through engaging daily programming.

- Program Execution: Lead and participate in individual, small-group, and large-group activities designed to spark interest and engagement.
- Calendar & Planning: Assist in developing the monthly activity schedule, including special resident outings, holiday events, and room visits for bed-bound residents.
- Documentation: Maintain accurate attendance records and assist with assessment documentation and progress notes as assigned.
- Resident Advocacy: Encourage residents to pursue self-initiated hobbies and provide necessary materials, such as crafts, books, or specialized media (Braille/audio).
- Communication: Act as a bridge between residents, families, and staff to ensure the social needs of our community are consistently met.
- Logistics: Assist in arranging transportation for outings and medical appointments when necessary.

- Experience: Previous experience in a long-term care facility is preferred, but we are looking for the right heart and attitude!
- Skills: Ability to read and interpret policy manuals, basic mathematical skills, and a knack for practical problem-solving.
- Physical Ability: Must be able to stand/walk for long periods, push/pull equipment, and occasionally lift up to 50 pounds to assist with activity setups.
