Travel Physical Therapist Home Health jobs in Concord, CA and the Tri Valley area offer a 13-week assignment with Monday through Friday 8-hour shifts, averaging 36 to 40 hours per week. You will provide in-home physical therapy services, helping patients regain independence and function after injury, surgery, or health changes. Required qualifications include a current California license, with home health experience preferred.
Your responsibilities include evaluating patient needs, developing and implementing care plans, educating patients and families, and collaborating with an interdisciplinary team. You will document care and outcomes, drive to patients’ homes throughout the coverage area, and support clients in achieving their functional goals
