Travel Occupational Therapist Home Health jobs in Concord, CA and the Tri Valley area offer a 13-week assignment with Monday through Friday 8-hour shifts, averaging 36 to 40 hours per week. You will provide hands-on therapy and rehabilitation services in patients' homes, using Kantime EMR to document care and progress. Required qualifications include a current California license, with home health experience preferred.

Your responsibilities include conducting comprehensive assessments, developing individualized treatment plans, and educating patients and families on adaptive equipment and safety techniques. You will collaborate with an interdisciplinary team to help patients achieve their highest level of independence in daily living activities
