Registered Nurse – Per Diem – Care Plan Assessment

Responsibilities
  • Perform face‑to‑face initial and ongoing nurse assessments per state license requirements and/or for Long‑Term Care Insurance assessments.
  • Obtain patient history, medical history, family history, social history, physical examination including vital signs, systemic assessment, pain assessment.
  • Functional assessment of Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
    • Activities of Daily Living (ADLs): assessment of bathing, dressing, eating, toileting, mobility.
    • Instrumental Activities of Daily Living (IADLs): assessment of finances, transportation, shopping, medication management.

Qualifications
  • Valid and active RN license in California.
  • Proven experience conducting thorough assessments and developing care plans.
  • Knowledge of regulations and compliance standards in non‑medical home care.
  • Strong communication and interpersonal skills to train and support caregivers.
  • Ability to work independently and as part of a collaborative team.
  • Empathetic, patient‑centered approach to care delivery.
  • Excellent organizational and time‑management abilities.
  • Commitment to continuous learning and professional development.
  • Knowledge of LTCI assessments is a plus.

Working Hours
  • Working hours may vary based on client needs; may include evenings and weekends.
