In this rewarding position, you will:

• Provide compassionate home-based care to patients recovering from illnesses, injuries, and chronic diseases.
• Collaborate with a team of physicians and oversee LPNs, CNAs, and aides in delivering patient care.
• Conduct patient assessments, document progress, and communicate changes to their physician.
• Educate patients and their families on care plans, medications, and home safety.
• Ensure that patient goals are met and coordinate discharge from services.

To join our team, you’ll need:

• An active, unrestricted Registered Nurse (RN) license in the State of Michigan (BSN not required).
• At least one year of clinical experience as a Registered Nurse.
• Current CPR Certification.
• Reliable transportation with a valid state driver's license and current car insurance.
