- Conduct ongoing assessments of the patient’s physical and psycho-social status.
- Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a comprehensive care plan.
- Provide education and interventions to manage pain and other physical symptoms effectively.
- Ensure availability of necessary equipment, supplies, and medications.
- Adapt the IDG plan of care as the patient’s/family’s needs evolve.
- Document patient/family services accurately and in a timely manner.
- Participate regularly in IDG meetings.
RN/LPN Requirements
- Active, unrestricted RN/LPN license in the State of Michigan.
- Minimum of one (1) year of relevant experience as an RN or LPN.
- Preferred:
One (1) year of management experience. - Preferred:
One (1) year of home health experience. - Current CPR Certification.
- Preferred:
Community health and a broad range of Medical-Surgical experience. - Reliable transportation with a valid state driver's license and current car insurance.
