Position: Licensed Practical Nurse LPN $10,000 sign on bonus FT and $5,000 PT

Rate increase to $36.19-$39.81 after 6 months of employment

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, offering short‑term rehabilitation, long‑term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on‑site dialysis and infusion therapy.

Sharing the campus are Gurwin’s Assisted Living community (Gurwin Jewish: Fay J. Lindner Residences), the independent living community (Fountaingate Gardens), and our Certified Home Health and Licensed Home Care agencies.

WE ARE CURRENTLY OFFERING A $10,000 SIGN‑ON BONUS TO ALL FULL‑TIME LPNs HIRED AND $5,000 FOR ALL PART‑TIME LPNs HIRED

Position

Full‑time Licensed Practical Nurses for evening and night shifts: 3:00 pm–11:15 pm and 11:00 pm–7:15 am (scheduled every other weekend).

Part‑time Licensed Practical Nurses for all shifts: 7:00 am–3:15 pm, 3:00 pm–11:15 pm, and 11:00 pm–7:15 am (scheduled every other weekend).

Requirements
  • Graduate of an accredited professional nursing program with a current license to practice in New York State.
  • Demonstrates interest in the special needs of geriatric patients and possesses an ability to integrate principles of geriatric nursing into nursing practice.
  • Ability to establish effective interpersonal relations with patients, staff and families.
  • Ability to communicate effectively in English, both orally and in writing.
  • Education in all areas of LTC/Subacute to ensure competency on all units in the facility.
Responsibilities
  • Participates in the nursing care of patients within the scope of the legal definition of practical nursing, working under the supervision and/or direction of a registered professional nurse.
  • Participates in the formulation and implementation of patient care plans to meet the specialized needs of geriatric patients under the supervision of an RN.
  • Administers patient care as assigned by an RN within departmental guidelines; may assume team leader, primary nurse or charge responsibilities under RN direction when required.
  • Directs and supervises non‑professional staff as assigned.
  • Observes, records and reports patient data as dictated by departmental policy.
  • Participates in unit nursing and inter‑disciplinary team activities.
  • Administers medications and performs nursing procedures as dictated by departmental policy.
  • Attends nursing and other institutional in‑service programs and participates in departmental committees as assigned.
  • Performs related and miscellaneous duties as directed; may be expected to perform unrelated duties in an emergency situation.
