A key function of this role is to provide direct support to households residing in Homeport units or Homeport developments. This role assists residents in identifying resources in the community to improve all aspects of their financial, physical and mental health. This role provides on-going information, support and referrals to households to assist households in decision making, goal setting and achieving overall stability.

Duties

and responsibilities
  • Serves as coordinator for Homeport residents and community direct services agencies.
  • Serves as a resource to Property Managers and residents regarding social service agencies.
  • Maintains current knowledge of industry trends and best practices to develop and implement practices that support community stability.
  • Participates and supports community programs.
  • Develops and implements programs to lower resident turnover.
  • Promotes resident participation in Homeport’s Financial Capabilities programs.
  • Maintains records of service delivery and prepares monthly statistical report.
  • Serve as a task instructor for Social Work interns.
  • Meets with individual residents and assists in the identification of community resources to address specific needs and issues and assists in the development of individual goal plans.
  • Maintains knowledge base of social service agencies, small businesses, government agencies and faith-based organizations within the geographical region to effectively link residents to resources.
  • Complete all required documentation (reports, case notes, etc.) and review with the Program Manager.
  • Meets with property managers to troubleshoot resident issues and assist in resolution.
  • Assists Homeport in determining appropriate resolution to resident problems and issues if requested.
  • Participates in community outreach events for Resident Services.
  • Collaborate with other Homeport staff to accomplish departmental goals.
  • Supports the obtainment of the annual departmental and organizational revenue and expense goals

Qualifications

Education and Experience

Required Bachelor’s degree in Social Work, or a related field, from an accredited school. Prefer Graduate degree.

  • Prior experience counseling clients from diverse backgrounds and issues.
  • Displays versatility in securing a diverse network of community and resident relationships that are supportive of Homeport’s goals and mission
  • Ability to work collaboratively and effectively in cross-cultural settings and with a wide variety of organizations
  • Ability to meet deadlines; work well under pressure; and communicate clearly, professionally, and personably with co-workers, customers, partners and vendors
  • Demonstrated ownership of and accountability for achieving departmental and organizational goals while demonstrating a team and customer service-oriented approach
  • Solid interpersonal skills and self-awareness
  • Strong interpersonal and negotiation skills
  • Demonstrated use of analytical skills
  • Solid ability to engage in independent field work and work collaboratively across Homeport departments
  • Strong time management and organizational skills
  • Proficient in using Microsoft Office tools
  • Holds and maintains Ohio driver’s license and maintains a good driving record
Licensing and/or Certification Requirements:

Social Work License preferred

This position requires a reliable, personal means of transportation.
