Essential Functions- The duties and responsibilities described are not a comprehensive list and additional tasks may be assigned from time to time, as the scope of the job may change as necessitated by business demands of the Hospice Admissions manager establishes, re-evaluates, promotes, and execute the admissions process from referral to patient admission.
- Oversees the Admission Team under the guidance of the associate clinical director, to ensure that the referral/admission process is performed in a smooth and coordinated manner. Serves as a resource to the Admissions team and works closely with hospice medical director.
- Responsible for timely referral visits and admissions in collaboration with scheduler; that referral visits are assigned appropriately taking into consideration staff, geographic location and needs of the patients; ensures timely follow up on all pending patients. Transfer of patients from Admissions to Patient Care occurs in a smooth and coordinated manner.
