The Registered Nurse (RN) will provide care and services for a group of hospice patients, visiting patients at private homes, assisted living facilities, and hospitals. Responsibilities include completing hospice admissions and patient visits as needed, answering calls from patients, families, and customers, making nursing visits for symptom management, deaths, and admissions, and advocating for hospice services. This is an after-hours on‑call position working seven days/nights on and seven nights/days off.
Shift schedule: Friday, Saturday, Sunday, 12‑hour shifts.
Qualifications- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license
- Admissions and on‑call experience preferred
- Works well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Works closely with patients on a 1‑to‑1 level while receiving support from a patient care team
- Can work independently, handling phone, email, written communication, meetings with diverse healthcare professionals, and understanding of hospice rules and regulations
- Strong time‑management skills to manage visits, documentation, meetings, and urgent needs by prioritizing and triaging multiple concerns
- Ability to learn an electronic medical records system
- Willingness to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area
