The Patient Care Resource Manager (PCRM) is a nurse case manager that facilitates patients through the health care system, including pre‑admission through post‑discharge coordination of care, resources, and services using the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput, improve care quality and efficiency, and ensure efficient resource utilization.
The PCRM identifies and removes barriers to progression of the plan of care for each patient, collaborates with physicians, nurses, and other health care workers to manage the patient transition to the next level of care, and serves as the central information source for third‑party payers, communicating the plan of care, medical necessity, and gaining approval for post‑discharge care. Through evidence‑based criteria, the PCRM assists physicians in ensuring the appropriate level of care for each patient.
Bachelor's level degree or equivalent combination of education and experience.
Required QualificationsBachelor’s degree; master’s preferred. Registered Nurse licensure required; five years of recent clinical experience applicable to the population preferred. Knowledge of clinical practice guidelines, computer databases, and payer protocols desired.
