Care Management Coordinator - Field - OhioRISE Central/SE Ohio

This full‑time, field‑based telework position requires traveling throughout Central and Southeast Ohio to support members’ behavioral health needs. Applicants must reside in the region, typically within the specified counties, and be willing to travel up to 50% of the time.

As a Care Management Coordinator, you will evaluate, plan, and coordinate medical and behavioral health services for children and families involved in multiple child‑serving systems. Your role will include clinical case management, quality improvement, and collaborating with a multidisciplinary team to achieve optimal member outcomes.

Evaluation of Members

• Conduct comprehensive assessments of referred members using care‑management tools to determine needs and eligibility.
• Identify high‑risk factors and appropriate service referrals.
• Monitor and implement care‑plan activities, tracking progress.

Enhancement of Medical Appropriateness and Quality of Care

• Collaborate with case managers, supervisors, medical directors, and external programs to overcome barriers and achieve goals.
• Participate in case conferences for multidisciplinary review.
• Escalate quality‑of‑care issues through established channels.
• Utilize negotiation and motivational interviewing skills to secure necessary services and promote member engagement.
• Provide coaching and support to empower independent health decisions.
• Serve as a single point of contact, addressing immediate care gaps.

Monitoring, Evaluation, and Documentation of Care

• Apply care‑management and quality‑management processes in accordance with regulatory and company policies.

• Minimum 2 years’ experience in behavioral health, social services, or related human services.
• Proficiency with personal computers, multi‑system navigation, and Microsoft Office Suite.
• Experience in children’s mental health, child welfare, developmental disabilities, juvenile justice, or public‑sector behavioral health.
• Experience in family systems, community resources, case management, child protection, or child development.
• Knowledge of Ohio delivery systems and community resources.
• Reliable transportation and willingness to travel within the region up to 50% of the time.
• Availability to work beyond Monday‑Friday, 8 a.m. – 5 p.m., as needed.
• Must reside in Ohio.

• Managed‑care experience.
• Medicaid experience.

Bachelor’s degree required; a non‑licensed master’s level clinician is also acceptable. Degrees in behavioral health, human services, health services, or public health are preferred.

Full‑time, 40 hours per week.

Hourly rate: $21.10 – $36.78, depending on experience, education, and geography.
