Position: COMMUNITY-BASED CLINICIAN

Community-Based Clinician at Youth For Christ USA Inc.

Position Summary

Supervisor:
Director of Clinical Services and Community Based Programming.

Goal:
To provide high quality, trauma‑informed & responsive therapeutic services and support to Gracehaven Community Based Team clients and their families, utilizing evidence‑based therapies and treatment modalities.

Key Objectives
  • Provide individual and family‑based clinical services to youth who are at risk and/or have been exploited and are active clients engaged in the Gracehaven continuum of care/services.
  • Assess youth referred for Community Based Therapeutic Services and make treatment and level‑of‑care recommendations in accordance with diagnosis and medical necessity.
  • Develop and implement individualized therapeutic treatment plans in accordance with the youth's diagnosis and treatment recommendations.
  • Provide on‑call coverage and crisis management consultation and intervention services to youth and families when deemed necessary.
  • Work in conjunction with the Director of Clinical Services & Community Based Programming to identify and implement an evidence‑based treatment model for Therapeutic Group Home and Community Based Treatment Services.
  • Contribute to and foster a trauma‑informed & responsive organizational culture.
  • Maintain all compliance standards.
  • Actively participate in supervision & ongoing professional development training to foster clinical skill development and enhance Gracehaven therapeutic services for youth & families who are at risk and/or have been exploited.
Clinical Responsibilities
  • Establish therapeutic rapport and relationships with Gracehaven Community Based Services clients and families.
  • Assessment:
    Develop a comprehensive clinical formulation based on a diagnostic assessment to establish medical necessity for services s includes an interview with the client, record review, observation, diagnostic impression, level of care, and individualized treatment recommendations.
  • Treatment Planning:
    Facilitate collaborative development of an Individual Service Plan (ISP) with the client, family, and other service providers; monitor implementation; regular review and update of service recommendations and treatment goals.
  • Therapy:
    Provide behavioral health services—including individual, group, and/or family therapy—to adolescents and families based on mutually agreed treatment goals within the Individualized Treatment Plan (ITP).
  • Discharge Planning:
    Participate in quarterly Multidisciplinary Progress Review and Maintenance/Discharge Planning Meetings; advocate for appropriate level of care and services in the least restrictive environment necessary.
  • Documentation:
    Provide accurate and timely documentation of service delivery and client progress in accordance with treatment goals and regulatory guidelines.
  • Communication &

    Collaboration:

    Maintain effective verbal and written communication with clients, families, Gracehaven service team, and interdisciplinary team members.
  • Crisis Intervention Services:
    Participate in a monthly on‑call rotation; provide crisis management consultation, risk assessment, and intervention when needed; identify urgent/emergent mental health symptoms and make appropriate referrals.
  • Assist client, family, and Gracehaven service team in developing effective safety and/or behavioral intervention plans.
Trauma‑Informed Organizational Culture Requirements
  • Demonstrate personal and professional accountability and support the philosophy, goals, and mission of Gracehaven.
  • Share information and training resources that support trauma‑informed/responsive practices with staff.
  • Support a trauma‑informed and responsive team ethos by modeling, coaching, and reinforcing symptom‑management techniques.
  • Utilize therapeutic, trauma‑informed/intervention techniques before, during, and after crises to support safe client de‑escalation and stabilization.
  • Maintain an active Ohio license in good standing with the State of Ohio Social Work and Counselors Board.
  • Obtain and maintain Dialectical Behavioral Therapy Certification and Gracehaven Level II QRTP-approved Trauma Model.
