Job Title: Social Worker - LSW

Department: Ambulatory Services | Maternal Fetal Medicine at Outpatient Care East

Responsibilities
  • Provide support and guidance to participants of the Moms2B Program, which helps pregnant women have healthy pregnancies, deliveries, and babies in a diverse low-economic area of Columbus with high infant mortality rates.
  • Assist program participants in navigating community and health care services; coordinate Moms2B sites; work within a multi-disciplinary team including students, interns, and volunteers.
  • Serve as a liaison to The Ohio Collaborative to Prevent Infant Mortality; use case finding techniques to identify and recruit pregnant women into early enrollment for prenatal care.
  • Provide enrolled participants with educational and intervention counseling, materials, and access to prenatal, postpartum, and inter-conception care; guide access to community resources, education, outreach, home visits, and referrals.
  • Direct enrollment of health and developmental education classes; facilitate and coordinate appointments with agencies and providers to ensure timely delivery of referrals.
  • Collaborate with participants to develop follow-up plans to meet present needs and address future needs; advise individuals and families in navigating the health care system.
  • Research and engage with available services and programs external to OSU for participants; attend and represent OSU at community meetings.
  • Assess, maintain, and coordinate up-to-date resources and materials used in the program; maintain communication with local area agencies and staff to share information, activities, and resources.
  • Collect, analyze, and compile research data for inclusion in progress reports, publications, and for dissemination at workshops and conferences.
Minimum Qualifications
  • Master s Degree in Social Work.
  • State of Ohio Social Work licensure (LSW).
  • 0 years of relevant experience required. 0-2 years of relevant experience preferred.
  • Experience in program planning and administration in a community-based program aimed toward a diverse low-economic population.
  • Experience in the organization of volunteers and program materials desired.
  • Demonstrated awareness of diversity issues and provides culturally sensitive services to clients preferred.
  • Requires successful completion of a background check. Selected candidate may be requested to complete a pre-employment physical including a drug screen.
