We are seeking a skilled Nurse Practitioner to join our healthcare team, providing comprehensive care across primary care, geriatrics, pain management, and hospice settings.
- Conduct physical examinations, assessments, and develop individualized treatment plans.
- Evaluate pain management needs, infectious diseases, and chronic conditions such as diabetes and cardiovascular issues.
- Assist with diagnostics, document interactions in EMR/EHR systems, and educate patients on health topics.
- Collaborate with multidisciplinary teams for discharge planning and case management.
- Maintain current knowledge of medical terminology and coding (CPT/ICD).
Requirements include a valid NP license, ability to manage complex cases, proficiency with diagnostic evaluations, and excellent communication skills. This in-person role offers 24 hours per week in a dynamic environment focused on improving patient outcomes.
