Overview
0.5 FTE year round with overall flexible work schedule but must meet the needs of school reintegration. Tuesdays are required. No weekends or holidays.
Job Description SummaryCollaborates with the patients and families, internal staff, and local school district personnel to facilitate successful school services and re‑integration for patients. Provides advocacy and coordination to families for recommended school settings, modifications, and accommodations.
Essential Functions- Serves as a liaison with school systems for patients, families, and community partners involved in providing education services.
- Coordinates education services and participates in family‑centered patient care for all patients, facilitating consent and communication with the home school district for each patient.
- Provides consultation and advocacy for families and school personnel in the process of special education referrals and development of individualized education programs (IEPs) or 504 Plans.
- Maintains regular contact with patient, family, school staff, and care providers to monitor academic progress and provides intervention as needed.
- Collect necessary data and documentation for ongoing program evaluation.
- Provides training and educational materials for families, school personnel, patient care staff, and other stakeholders.
Master's Degree in Social Work or Education, required.
Licensure Requirement- Valid professional license as educator, social worker, or other appropriate professional, required.
- Valid Ohio driver’s license and proof of auto insurance as required by hospital policy and position‑specific requirements. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain qualification of insurance guidelines.
- Effective initiative and the ability to work independently and collaboratively.
- Working knowledge of child development.
- Proficient with Microsoft Office and interactive computer technologies.
- Excellent customer service, communication, and interpersonal skills.
- Effective analytic and organization skills.
- Working knowledge of special education services.
Three years of advocacy experience in a school setting or hospital environment, preferred.
