Responsible for verifying new medication orders, checking prepared medications, overseeing dispensing from the pharmacy, and completing clinical patient review for safety and accuracy. Providesveck high‑quality, patient‑centered, evidence‑based medication services to all patients. In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, families, nurses, physicians, advanced practice providers,etek etc. Screens for drug‑drug, drug‑herb/dietary supplement, drug‑allergy, drug‑food, drug‑disease state, and drug‑laboratory interactions.
Obtains and maintains clinical privileges, if applicable. With privileges, adjusts medications based on route of administration, monitors renal, hepatic, antithrombotic indications, and hematologic parameters, orders and modifies laboratory tests related to medication therapy, and adjusts total parenteral nutrition in conjunction with a dietician, if applicable. Participates in Emergency Response efforts, including Code Blues and Stroke Alerts, and engages in appropriate hospital and departmental committees.
Summary
This position coordinates all aspects of patients’ pharmaceutical needs at The Ohio State University Wexner Medical Center. Duties include medication order approval and distribution; assurance of appropriate dose, route, frequency, and duration of therapy; clinical profile review for safety and accuracy; patient medication profile review for drug‑drug interactions, duplication of therapy, drug allergies, and regimens; coordination with buffered physicians, advanced practice providers, and nurses;
use of clinical pharmacy privileges to promote optimal medication practices; and contribution to Emergency Response efforts. The role also oversees technicians, undergraduate and graduate students, and provides educational programs for hospital staff.
