• Assist with activities of daily living (e.g., personal hygiene) as outlined in the patient’s plan of care.
• Apply basic infection control principles and procedures.
• Maintain a record of the Home Health Aide plan of care for each patient and provide updates to the assigned case manager and the patient’s family.
• Attend required in‑services and meetings.

  Ms (What Matters to the patient, Medications, Mentation, Mobility) to deliver age‑friendly care.

• Must have and maintain a vehicle for work, a current driver’s license, a good driving record, and proof of insurance.
• Hold a Home Care Aide certificate.
• Have a minimum of one year of full‑time experience providing direct client care in an institutional setting (hospital or nursing facility).
• Have one year of full‑time experience within the last five years in direct client care in an agency setting.
