• Responsible for verifying new medication orders, checking prepared medications, overseeing dispensing from the pharmacy, and completing clinical patient review for safety and accuracy.
• Provide safe, efficient, high quality, patient-centered, and evidence-based medication services to all patients.
• Collaborate with pharmacists and pharmacy technicians to answer drug information questions for patients, family members, and hospital staff (pharmacists, nurses, physicians, advanced practice providers, medical students, and others).
• Screen for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food, drug-disease state, and drug-laboratory interactions.
• Obtain and maintain clinical privileges, if applicable, and use them to adjust medications, monitor laboratory parameters, and modify therapies (including Total Parenteral Nutrition) in conjunction with a dietitian, if applicable to the role.
• Participate in Emergency Response efforts (e.g., Code Blues and Stroke Alerts) and engage in relevant hospital or departmental committees.

This position coordinates all aspects of patients' pharmaceutical needs at The Ohio State University Wexner Medical Center. Duties include medication order approval and distribution, ensuring appropriate dose, route, frequency, and duration of therapy; clinical profile review for safety and accuracy; patient medication profile review for interactions and duplication; coordination of patients' pharmaceutical needs with physicians, advanced practice providers, and nurses; use of clinical pharmacy privileges to promote optimal medication practices;

and contribution to Emergency Response efforts with the healthcare team. The role requires the ability to provide care appropriate to the age of the patients served. The position also oversees technicians, undergraduate and graduate student teaching, and provides educational programs for hospital staff.

• For Hire: Licensed or eligible for pharmacist licensure in Ohio.
• Ongoing: Performs professional, oversight, and teaching functions as outlined above; maintains competency and licensure expectations of a staff pharmacist; obtains and maintains required clinical privileges for the role.
