In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, families, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy students, and other associate hospital personnel. Approves new drug orders, verifies chemotherapy notes, informed consent, and processes non‑formulary requests. Screens for drug‑drug, drug‑herb/dietary supplement, drug‑allergy, drug‑food, drug‑disease state, and drug‑laboratory interactions.

Participates in appropriate hospital and/or departmental committees relevant to the practice area.

This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to medication order approval and distribution; assurance of appropriate dose, route, frequency, and duration of therapy; patient medication profile review for drug‑drug interactions, duplication of therapy, drug allergy interactions, and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses.

The patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location. Must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served, medication dosing, and identification of pharmaceutical needs relative to the disease process and age. The role also includes supervising Pharm.

D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff.

• Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.
  
  D./MS) and residency; and/or equivalent experience preferred.
• Conducts experiential and/or didactic teaching of pharmacy students (including interns), pharmacy residents, and permanent staff.
• Provides professional instruction to hospital and clinical staff or other professional organizations.
• Participates and presents in intra‑departmental continuing education programs as well as professional presentations to local community groups, the College of Pharmacy, other hospitals and pharmacy departments, and professional meetings.
