Overview

IRP-Staff Pharmacist at The Ohio State University Wexner Medical Center

Responsibilities
  • Verify new medication orders, check prepared medications, oversee dispensing from the pharmacy, and complete clinical patient review for safety and accuracy.
  • Provide safe, efficient, high quality, patient-centered, and evidence-based medication services to all patients.
  • Collaborate with pharmacists and pharmacy technicians to be a resource for drug information questions for patients, family members, and hospital staff.
  • Screen for drug interactions and other medication-related risks; manage allergies, interactions, and special dosing considerations.
  • Obtain and maintain clinical privileges, if applicable; adjust medications based on route of administration, monitor laboratory parameters, and modify therapy as needed (including Total Parenteral Nutrition therapy with dietician, if applicable).
  • Participate in Emergency Response efforts (Code Blues and Stroke Alerts) and serve on relevant hospital/departmental committees.
Position Summary
  • Coordinate all aspects of patients' pharmaceutical needs; approve and distribute medication orders; ensure appropriate dose, route, frequency, and duration.
  • Perform clinical profile reviews for safety and accuracy; assess drug interactions and duplication; coordinate with physicians, advanced practice providers, and nurses.
  • Utilize clinical privileges to promote optimal medication practices; contribute to Emergency Response efforts; provide educational programs for hospital staff; supervise technicians and students.
  • Provide care appropriate to the age of patients served.
Minimum Qualifications
  • For Hire:
    Licensed or eligible for pharmacist licensure in Ohio.
  • Ongoing:
    Perform professional, oversight, and teaching functions; maintain competency and licensure; obtain and maintain required clinical privileges.
