Scope of PositionIn cooperation with other pharmacists and pharmacy technicians, this pharmacist acts as a resource and provides answers to drug‑information questions for patients, families, pharmacists, nurses, physicians, medical students, graduate pharmacy students and other associate hospital personnel both inpatient and outpatient. Approves new drug orders and screens for drug‑drug interactions, drug‑herb/dietary‑supplement interactions, drug‑allergy interactions, drug‑food interactions, drug‑disease state interactions and drug‑laboratory interactions (in cooperation with the Dietetics Department).
Participates in the pre‑, peri‑, and post‑transplant management of patients in the hospital and in the transplant clinic. Participates in appropriate hospital and/or departmental committees relevant to transplant.
This position functions to coordinate all aspects of patients’ pharmaceutical needs in abdominal transplantation. Duties include, but are not limited to: participation in the transplant clinic; medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug‑drug interactions, duplication of therapy, drug‑allergy interactions and appropriate regimens; medication reconciliation; coordination of patients’ pharmaceutical needs with physicians, mid‑level providers, and nurses.
The patient population includes adult kidney, liver, and pancreas pre‑ and post‑transplant recipients. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position also supervises Pharm.
D. students, technicians, undergraduate and graduate student teaching, and provides educational programs for patients and hospital staff (physicians, nurses and pharmacy staff).
