Specialty Practice Pharmacist
- Solid Organ Transplant at The Ohio State University Wexner Medical Center. This role coordinates all aspects of patients7 pharmaceutical needs in abdominal transplantation and supports transplant clinic activities.
- In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, families, pharmacists, nurses, physicians, medical students and other hospital personnel (inpatient and outpatient).
- Approves new drug orders and screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions.
- Participates in the pre-, peri-, and post-transplant management of patients in the hospital and in the transplant clinic.
- Participates in appropriate hospital and/or departmental committees relevant to transplant.
- Coordinates all aspects of patients7 pharmaceutical needs in abdominal transplantation, including participation in the transplant clinic and medication order approval and distribution.
- Ensures appropriate dose, route, frequency and duration of therapy; reviews patient medication profiles for drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens.
- Performs medication reconciliation and coordinates patients7 pharmaceutical needs with physicians, mid-level providers, and nurses.
- Serves adult kidney, liver and pancreas pre- and post-transplant recipients; demonstrates knowledge and skills to provide care appropriate to the age of the patients served; applies medication dosing and identifies pharmaceutical needs related to disease processes and age.
- Supervises Pharm.
D. students, technicians, and provides teaching and educational programs for patients and hospital staff (physicians, nurses and pharmacy staff).
- Functions to coordinate all aspects of patients7 pharmaceutical needs in abdominal transplantation.
- Includes duties such as participation in the transplant clinic, medication order approval and distribution, and ensuring appropriate therapy management.
- Requires ability to provide care appropriate to adult patients and to identify pharmaceutical needs related to disease processes and age.
- Responsible for supervising students and staff and providing educational programs for patients and hospital staff.
- Ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
- Ability to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.
- Experience supervising Pharm.
D. students, technicians, and coordinating educational activities is expected.
