Overview
Our Hospice is seeking a Registered Nurse Case Manager (RNCM) – Float, serving all 22 counties in our service area. As an RN Case Manager on our Float Team, you will provide compassionate, skilled care to a diverse caseload of patients alongside a supportive interdisciplinary team that includes Nurse Practitioners, Physicians, Aides, Chaplains, and Social Workers. You will play a vital role in guiding patients and families through one of the most meaningful times in their lives.

Flexibility, adaptability, and strong clinical judgment are key, as you’ll be on the go visiting patients throughout the community.
Schedule & HoursMonday–Friday – On‑call rotation required. Typical hours: 8:00 a.m.–4:30 p.m.
Competitive pay with additional $2.50 per hour float premium.
Responsibilities- Make routine Hospice nursing visits as assigned to patients in the home, extended care facilities, assisted living facilities, and hospitals. Complete assessment of the patient/family needs including physical, emotional, and bereavement status.
- Perform as case manager maintaining responsibility for assigned patients in a cost-effective and quality manner. Manages time, reimbursement of visits, pharmacy and medical supplies, and durable medical equipment.
- Teach, supervise and counsel patient/family care techniques including pain and symptom control, dietary instruction, use of equipment and observation techniques.
- Participate in on-call service to patient/family to insure availability on a 24 hour, seven day a week basis.
- Develop and implement the plan of care for each patient in collaboration with the patient, family and Interdisciplinary Group (IDG). Evaluate each patient/family as directed and makes appropriate changes to the plan of care.
- Communicate with and update other IDG team members as needed to coordinate and ensure quality care (e.g. Social Services, Home Health Aides, Volunteers, and Medical Director).
- Promote the Hospice benefit and provides education to all potential referring customers including but not limited to patients and family members, case managers, nursing facilities administrators and staff, physicians and physician office staff, and community groups.
- Maintain accurate records in accordance with the supervising organization’s policies and procedures in a timely manner (e.g. initial plan of care, comprehensive plan of care, and updates to the plan of care, daily documentation requirement, discharge summaries, death summaries).
- Associate or Bachelor Degree in Nursing (ASN/BSN)
- Current Registered Nurse License in Indiana
- One to three years’ experience in health care preferred
- Interest and ability in providing patient care in homes and facilities
- Ability to travel via your own vehicle
- Valid Indiana Driver’s License and car insurance
