Nurse Practitioner Value-Based Primary Care in Columbus, IN making $140k
Compensation:
- Base: $130,000+ (Flexible based on experience)
- Bonus: Up to $10,000 annually
- Sign-On Bonus Available
Schedule
- Monday Friday, 8am5pm
- No nights, no weekends
- Limited call: 1 week, 34x per year
- First call handled by call center
- NP is rarely contacted after triage
Practice Model
- Value-Based Outpatient Primary Care
- Geriatric population
- Small patient panel
- 1216 patients/day
- Dedicated Care Team:
- MA, Scheduler, Referral Specialist, Social Worker, Pharmacist
- Additional specialty support: Endocrinology & Podiatry
Role Overview
The Nurse Practitioner will provide comprehensive primary care to an older adult population in a value-based model. This role involves clinic-based care with occasional home visits and telehealth support. The NP will manage chronic conditions, develop treatment plans, educate patients, collaborate with an interdisciplinary team, and maintain thorough documentation aligned with risk-adjustment expectations.
Key Responsibilities
- Conduct patient assessments, physical exams, and diagnostics
- Manage chronic conditions (e.g., diabetes, COPD, CHF, dementia, debility)
- Develop individualized treatment plans aligned with patient goals
- Educate patients and families on medications, nutrition, disease progression, and prevention
- Collaborate with interdisciplinary team members to coordinate care
- Prescribe medications within state scope
- Provide care in clinic, patient homes, and via telehealth as needed
Requirements
- Masters degree in Nursing
- Board-certified NP (Adult, Adult-Gero, or Family)
- 2+ years Primary Care outpatient experience required
- Experience with value-based care (preferred)
- Must be able to practice autonomously
- No new grads
- Active NP license (state-specific)
- DEA (or eligibility)
- Experience with geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation (preferred)
- EMR experience;
Athena highly preferred
