Candidates must be located in Bartholomew, Brown or Jackson County.Field: This field‑based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands‑on engagement.

Responsibilities- Assist the responsible RN in identifying high‑risk members.
- Obtain clinical data as directed by the responsible RN.
- Assist the responsible RN in identifying members who would benefit from an alternative level of care or other waiver programs.
- Provide all collected information to the responsible RN, who verifies and interprets it, conducts additional assessments as necessary, and develops, monitors, evaluates, and revises the member’s care plan to meet the member’s needs.
- Participate in coordinating care for members with chronic illnesses, comorbidities, or disabilities as directed by the RN, and in conjunction with the health care team, to ensure cost‑effective and efficient utilization of health benefits.
- Make decisions based on the current needs of the member, applying an understanding of disease processes, terminology, and clinical guidelines. Nursing judgment is not required.
- Requires an LSW, LCSW, LMSW, or a license other than RN in accordance with applicable state law, and a Nursing Diploma, AS in Nursing, or a related field, with a minimum of 2 years of experience working with individuals with chronic illnesses, comorbidities, or disabilities in a Service Coordinator or similar role; or any combination of education and experience that provides an equivalent background.
- Current, unrestricted LSW, LCSW, or LMSW (or license other than RN as allowed by state law) in applicable state(s) required.
- MA/MS in Health or Nursing preferred.
- State‑specified certification based on state law and/or contract preferred.
- Travel to worksite and other locations as necessary preferred.
