Position SummaryThe Per Diem LPN provides direct nursing care to residents, supporting their safety and well-being under the supervision of RNs or physicians. This role offers flexibility and the opportunity to work in a dynamic healthcare environment.
Responsibilities- Provide direct nursing care in accordance with physician orders, care plans, and facility policies
- Administer medications and treatments, ensuring accurate documentation
- Monitor vital signs and resident responses to care
- Report changes in resident status promptly to the RN or physician
- Assist with admissions, discharges, and transfers as needed
- Supervise and support CNAs during shifts
- Ensure compliance with infection control, safety, and regulatory standards
- Complete required documentation before the end of each shift
- Maintain professionalism and adaptability in a fast-paced environment
- Graduate of an approved practical nursing program
- Current CPR/BLS certification
- Ability to pass background check and drug screening
- Long-term care or rehab experience preferred
- Reliable transportation and ability to accept short‑notice shifts
- Ability to stand, walk, bend, and assist residents throughout shifts
- Ability to lift up to 50 lbs with or without assistance
