RN Hospice - Full Time On Call (Columbus, GA) – Pro Health Home Health and Hospice

Job Summary
:
Provide nursing care to terminally ill hospice patients as needed, offer assistance and understanding to families within the home care setting and during bereavement, and work collaboratively as a member of the hospice team.

To perform nursing care, coordinate symptom management, monitor patients’ physical and emotional conditions, and maintain continuity of care through interdisciplinary collaboration.

Qualifications
  • Education:

    Graduate of an accredited school of nursing.
  • Licensure:
    Currently licensed as a registered nurse in the state.
  • Knowledge &

    Skills:

    Ability to work independently; make accurate and timely judgments; supervise others appropriately; respond to crises outside of a hospital; adapt to diverse social, racial, cultural, and religious contexts; complete hospice training program.
  • Experience:

    Minimum 2 years as a registered nurse; active patient contact within the past three years preferred.

  • Physical Requirements
    :
    Requires considerable physical effort, including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes, and maximal assistance in lifting and/or transferring a 100‑pound patient. Must possess sight/hearing or use adaptive devices to meet job demands and provide evidence of annual TB test and other required examinations.
  • Mental Requirements
    :
    Must be able to work independently, make judgments based on assessments and available data, act appropriately, maintain flexibility, innovate, possess good interpersonal skills, cope with mental and emotional stress, and demonstrate emotional stability.
  • Working Conditions
    :
    Must tolerate exposure to elements such as odors, blood, body fluids, excrements, adverse environmental conditions, and hazardous materials.
  • Transportation
    :
    Must hold a current valid driver’s license, auto liability insurance, and reliable transportation.
Essential Functions
  • Regularly assess and reassess the nursing needs of the hospice patient.
  • Provide dietary counseling.
  • Deliver hospice nursing services, treatments, and preventive procedures.
  • Initiate nursing procedures appropriate for the patient’s hospice care and safety.
  • Observe signs and symptoms and report any unexpected changes in the patient’s physical or emotional condition to the physician and inter‑disciplinary team members.
  • Teach, supervise, and counsel the patient and family about providing care for the patient.
  • Supervise and train other nursing personnel.
  • Develop and re‑evaluate the patient/family care plan in conjunction with the interdisciplinary team to meet needs and maintain continuity of care.
  • Perform specific nursing procedures as needed (e.g., treatments, symptom management) following doctor’s orders.
  • Attend team conferences.
  • Maintain required records.
  • Follow hospice policies and procedures, observe confidentiality, and safeguard patient information in compliance with HIPAA regulations.
  • Always communicate to the supervisor if unable to meet a patient’s need or perform a procedure.
  • Participate in the on‑call system and provide coverage when unavailable for assigned duties.
  • Maintain skills and knowledge.
  • Work with the interdisciplinary team concept of patient care.
  • Coordinate the implementation of the care plan for patients residing in SNF, NF, ICF, or MR.
  • Organize work schedule and utilize time management to attend all required meetings.
  • Comply with agency infection control policies and protocols.
  • Assist with orientation, teaching, and training as requested.
  • Perform other duties as assigned by the Director.
