Job TypeFull-Time (40 hours per week)
DaysMonday through Friday, 8am to 5pm, with a 1-hour lunch break
PopulationMales and females, 18 years of age or older, with a mental health disorder and/or addictive diseases.
Salary$31,500 per year
Under general supervision, the candidate will do the following- Receive and follow up on referrals.
- Make appointments for intake.
- After intake, complete referral to New Horizons Behavioral Health MAT-SOR Program.
- Complete Medication Pharmacy Grant Rx Payment Voucher to Genoa Healthcare.
- Complete DHP Authorization for Release of Protected Health Information (PHI).
- Complete Midtown Recovery Release of PHI
- Contact MAT individuals weekly with documentation into WITS system
- Assist with any needs for MAT, doctors appointments, getting medication, and assistance with receiving treatment.
- Coordinate with provider and nurse to address any concerns they may have.
- Attend meetings for MAT to update on progress.
- Procure and sign out Narcan.
- Conduct evidence-based group with individuals who have been identified with a substance abuse disorder.
- Document daily notes on progress in groups and provide resources in the surrounding community that aid in maintaining sobriety.
- Aid in increasing coping and basic living skills.
Maintain accurate confidential records of client interactions and progress. Prepare reports and documentation in compliance with organizational and regulatory requirements.
CollaborationCollaborate with a multidisciplinary team, including mental health professionals, to ensure holistic care for clients. Attend team meetings and case conferences to contribute a peer perspective to the overall care plan.
Minimum QualificationsHigh School Diploma with 3 to5 years of experience in behavioral health
-or- completion of a bachelor's degree in the social sciences (Social work, psychology, sociology, counseling, or a related discipline) with 1 year of full-time experience working with individuals with mental health disorders and/or addictive diseases. If candidate has a Bachelors, it MUST be in the above named fields to be MHC eligible.
A bachelor's degree and experience in working with individuals with mental disorders and/or addictive diseases is preferred.
