  • Facilitate case management sessions, including treatment goal setting, discharge planning, and collaboration with referral sources and external providers.
  • Educate clients on mental health conditions, treatment options, recovery strategies, and coping skills.
  • Coordinate care for all appropriate support services, including referral sources, family members, and other support providers.
  • Participate in treatment team meetings to review client progress and coordinate holistic care.
  • Provide crisis intervention and participate in a rotating on-call schedule to support patients in crisis after hours, as needed.
  • Maintain accurate, timely clinical documentation in alignment with organizational, state, and CARF standards.
  • Collaborate with psychiatrists, case managers, and other providers to meet co-occurring psychiatric and substance use needs of patients.
  • Develop and maintain a therapeutic alliance that is collaborative, individualized, and adaptable to evolving clinical needs.
  • Promote the organization’s image in a professional, supportive, and educational manner within the community and with referral partners.
  • Stay informed on current clinical practices, regulatory changes, and advancements in the behavioral health field.
  • Demonstrate a commitment to clinical excellence, as measured by outcomes such as patient retention, treatment goal achievement, successful program completion, and transitions between levels of care.
  • Obtain necessary secondary signatures on documentation from licensed supervisors when applicable.
  • Attend all required staff meetings, trainings, and professional development opportunities.
  • Perform other duties as assigned.
