Compensation is based on total years of experience in previous clinical roles. In this role, you ll care for patients in Allen, Noble, Dekalb, & Whitley Counties.

You will be working 24 hours each week with weekend rotations.

• POSITION OBJECTIVE:
  Performs nursing procedures and care to assigned terminally ill patients by assessing needs and implementing nursing care plans under the coordination of the RN Case Manager.
• ESSENTIAL FUNCTIONS:
  
  Must be capable of functioning independently in the patient's setting within the scope of the Registered Nurse. Delivers nursing care to patients, including pain/symptom management, teaching, direct care, case management, referral, advocacy and monitoring.
• Supervises clinically the care delegated to LPNs, Home Health Aides, CNAs, Companions and/or Homemakers. Instructs LPNs, Home Health Aides, and/or CNAs on specific patient care procedures within approved policies.
• Maintains a therapeutic relationship with patients/families and adheres to privacy practices. Documents findings accurately and promptly according to agency procedures.
• Follows orders from patients’ physicians and communicates changes in condition to the physician and other personnel.
• Assists with the Interdisciplinary Plan of Care and ensures emotional and physical comfort and safety of patients.
• Observes, records, and reports symptoms and changes; orients patient/family to hospice services; respects cultural and religious traditions.
• Maintains professional boundaries and productivity; participates in on-call rotation and agency committees; seeks supervision and educational opportunities.
