Position Summary

This care is provided to adults who reside in skilled nursing, assisted living facilities, and private residences through a network of healthcare providers including physicians, nurse practitioners, physician assistants, psychologists, licensed clinical social workers, and clinical and administrative support staff.

Clinician Support Specialist Responsibilities:

  • Enhance the performance of the clinical care team by supporting and facilitating clinical services with assigned providers.
  • Work in the field with the assigned provider as directed by Lead Clinician Support Specialist or Manager/Supervisor; may be asked to travel up to 60 miles from home.
  • Work 8‑hour dayshift, Mon‑Fri. Fuel card available for commutes.
  • Provider Support
    • Ensure the clinical provider in the assigned building can focus on patient care each day.
    • Ensure each patient is seen as medically necessary per protocol.
    • Serve as a buffer between provider and families/staff to facilitate proper patient management.
    • Document appropriate encounter and time for CCM and BHI services.
    • Update scheduling rosters daily and assist with day‑of changes for visits due to acute needs, hospitalizations, admissions, or discharges.
    • Prepare clinical documentation for assigned providers each day.
    • Complete basic opening of encounter note per discipline protocol.
    • Perform medication reconciliation on all scheduled patients each encounter.
    • Enter facility chart data, lab results, and other diagnostic study results into patient encounter notes and assign to provider as per guidelines.
    • Assist with collection and sorting of all clinical paperwork needed for clinician review, ensuring completion each day.
    • Host telehealth sessions for providers as assigned by manager/supervisor, both routine and acute visits.
  • Facility Support
    • When scheduling is an expectation, share patient schedules with key facility staff as needed.
    • Assist with triage needs as directed by assigned provider, whether in or out of the building.
    • Assist with triage documentation as needed.
    • Ensure that all needed paperwork is routed to Eventus providers, office, or families.
    • Assist with consents for any services required.
    • Attend stand‑up meetings to report any necessary information from the facility to Eventus providers, and vice versa if allowed.
    • Assist with discharge planning of clinical services as needed.
    • Check in and out each day with facility staff regarding patients seen, status, etc.
  • Patient Support
    • Maintain patient confidentiality at all times.
    • Document appropriate encounter and time for CCM and BHI services.
    • Check all forms of communication several times daily to ensure patient needs are met.
    • Inform patient and family of recent visits, new orders, and defer to provider when needed.
    • Communicate transitions of care to CCM team to enhance patient care and education.
  • Additional Duties and Responsibilities
    • Adhere to Eventus Whole Health standards, policies, and procedures, including HIPAA regulations.
    • Follow Eventus dress code: clean, wrinkle‑free, matching scrubs, closed‑toed footwear, name badge.
    • Report to Lead Clinician Support Specialist for additional assignments if duties are completed ahead of schedule.
    • Seek further training if uncertain how to proceed with any assignment.
Qualifications / Education
  • High School Diploma or equivalency.
  • Valid driver’s license and reliable transportation; expected to travel up to 1 hour from home.
  • Typing speed of at least 45 WPM.
  • Certified Clinician Support Specialist degree a plus.

  • Medical scribing experience preferred.
